The Greenwich Community Health Improvement Partnership (GCHIP) is a coalition of community leaders, health and wellness professionals and social service providers who advocate for the physical and mental health of our communities. Through collaboration across many diverse sectors, we share resources and work to identify, prioritize and improve the health of communities by providing educational programs and prevention services that promote health equity.
Improving the health of a community is critical to ensuring the quality of life of its residents and fostering sustainability and future prosperity. Health is intertwined with many different facets of our lives where we work, live, learn, and play all impact our health. Understanding the current health status of a community and the social determinants of health is vital to identifying health priorities for planning programs, services and coordination across partner organizations.
