GCHIP BUILDING HEALTHY COMMUNITIES SINCE 2003
A Community Health Needs Assessment (CHNA) is a comprehensive process used to identify the health needs of the community. This process is conducted as part of a larger goal of measurably improving the health and wellbeing of its residents. Understanding the current health status of the community through the CHNA process identifies priorities for future planning and funding.
In 2025, Greenwich Hospital, in collaboration with the GCHIP and the Council of Community Services (CCS), completed a CHNA to determine the health needs of residents living in Greenwich, CT and the Sound Shore communities of Westchester County (NY). The CHNA included the development of a comprehensive Community Health Improvement Plan (CHIP), a 3-year plan to address the major unmet needs in the region. Greenwich Hospital provides support for GCHIP and CCS to meet community needs and assist with carrying out the CHIP.
The 2025 CHNA process includes collecting primary and secondary data, asset mapping, conducting surveys, and community engagement. After the data was analyzed, numerous community partners collaborated to identify four health priorities: Promoting a Culture of Health, Access to Care & Services, Mental Health & Wellness, Supporting Healthy Generations. A CHIP was then developed to address the four priorities; each priority has its own subcommittee where members meet monthly to work together on implementing the plan.
